I keep getting error when trying to add Radio Buttons and Checkboxes to Cart Items / Products. I wish to assign Radio and Checkboxes to addons

I need to make checkout addons more accessible:

For example:

Awesome Box

Variants: 

Size (required) -> Picklist
a. 10" (+$10.00)
b 14" (+$20.00)

Color (required) -> Radio Button
a. Red (+$1.25)
b. Blue (+$1.00)

Addon/Upsells (optional checkboxs)
a. Yellow Ball (+$1.50)
b. Red Ball (+$2.50)
c. Another Awesome Box (+$10.00)

So Customer order summary  Awesome Box > 10" option > Red > with yellow ball, redball  and Another Awesome Box.

Order Summary
10" Red Awesome Box ... $11.25
Red Ball ... $2.50
Yellow Ball ... $1.50
Another Awesome Box ... $10.00

Sub Total: 25.25
